The website feature real-life accounts of issues such as effect on work and education, social life and relationships, consulting health professionals and treatment.

NB: DO NOT USE THIS FORM FOR ANYTHING OTHER THAN REQUESTING PRESCRIPTIONS FOR MEDICATION THAT YOU HAVE BEEN PRESCRIBED PREVIOUSLY BY A G.P.

PRESCRIPTION REQUESTS CAN NO LONGER BE TAKEN BY TELEPHONE.

IF YOU HAVE ENTERED INTO AN AGREEMENT WITH A PHARMACY TO COLLECT YOUR PRESCRIPTION ON YOUR BEHALF YOU SHOULD ALLOW FOUR WORKING DAYS BEFORE COLLECTING YOUR MEDICATION FROM THE PHARMACY  - YOUR PRESCRIPTION WILL NOT BE AVAILABLE AT THE SURGERY  

Please note that your prescription will be sent to the pharmacy you are coded for.  If you need to change this please contact the practice to speak to a member of the Reception team or contact the pharmacy and sign a new agreement with the pharmacy of your choice.

 

NB - If you have run out of your medication and need it the same day you should not use this form.  Please contact the practice in person or by telephoning between 11 am and 12 noon.

Patients receiving repeat medication on a long-term basis will have a list of this medication printed on the right hand side of their prescription. This list can then be handed in, or posted to the surgery when a further prescription is required. If a stamped addressed envelope is included, we will return your prescription by post.

If the medication is not listed on the right hand side of your prescription then either a handwritten request or an on-line request is required.

Prescriptions can be collected 2 working days after you have made your request unless you have asked a pharmacy to collect it on your behalf (see above)
